IMPORTANT PRE-EMPLOYMENT QUESTIONNAIRE: If you think you would like a position working with individuals with Developmental Disabilities, please answer the following questions to see if this type of work is for you!

  1. Do you enjoy working with children?

  2. Do you know what Developmental Disabilities are?

  3. Are you comfortable working in someone else's home?

  4. Are you willing to work a flexible schedule? (many families need workers evenings and weekends.)

  5. Do others consider you a responsible person?

  6. Do you pride yourself on your professionalism?

  7. Are you comfortable with taking the role of a caregiver?

  8. Are you comfortable with being paid once a month?

  9. Are you comfortable speaking/interviewing with other adults?

  10. Are you fluent in English? (Bilingual Spanish speakers also needed.)

If you have answered "YES" to all 10 questions, you may be the right person for employment with H.O.P.E. Group! Please keep in mind families are seeking enthusiastic, honest and compassionate individuals to work with their family members.

  • EMPLOYMENT REQUIREMENTS: Please recognize/understand that you must be fluent in English and/or Spanish( if you're working with a Spanish speaking client) in order to complete the Orientation and Training process required by the State of Arizona and be able to work with English speaking clients.  If you do not speak English it will be difficult for you to work with a client who only speaks English.  If you wish to work with a family member/friend who speaks the same language you speak, but it is not English or Spanish, please contact the office about one-on-one language specific trainings. Before you are hired by H.O.P.E. Group you must complete the following: 

1.  Article 9 and Health and Safety tests (Information packets will be given and tests will be administered on training and orientation day) 

2.  C.P.R and First Aid Training

You may take one of the classes we offer in our office - please call us at (480) 610-6981 to schedule. Or, if these dates do not work for you, you may go to any instructor lead class.

IMPORTANT: You must have adult CPR and First Aid, but we prefer a combination of adult/child.  We will not accept certifications from an online class; your class must be instructor lead.

If you choose to take the HOPE Group class, it will be free to you and your card will be property of HOPE Group.

We strongly recommend that you mail or fax these prior to your hiring orientation, to provide ample time for verification.

5. Attend an A.B.A. Training & New Hire Orientation: Trainings are held at the HOPE Group Office. Please call as far in advance as possible to schedule this, as classes fill up quickly.

You may not bring children to the training or orientation. You will be asked to reschedule your class for a more convenient time.

List of Documents to bring to the Hiring Orientation. * (NOTE: if we can not send you an initial hire packet in the mail, or you have not received  it, before your orientation date, please print documents 1-9  to bring with you. All forms must be completed and signed)

  3. W-4 federal tax form**

  4. A-4 Arizona tax form**

  5. I-9 form**

  6. Direct deposit form (optional)

  7. DDD policy on fraud and abuse 

  8. Employment verification form

  9. Training agreement

  10. Drivers license or state ID

  11. Social security card or birth certificate 

  12. Proof of CPR training (online classes not accepted) 

  13. Proof of first aid training (online classes not accepted)

  14. Fingerprint clearance card (if you do not have one, we will fingerprint you at the orientation.)

* You must submit all completed and signed forms, as well as complete the requirements prior to receiving a hire date.

Please note: 

  • The ABA training and hiring orientation will take about 8 hours in total.   You will be filling out employment forms and going over the rules and regulations.
  • If you submit timesheets prior to receiving your hire date from us, you will not receive payment for the hours worked.
  • In accordance with H.O.P.E. Group’s monthly reimbursement schedule from DDD, we pay on a semi-monthly basis.
  • Pay Rates: Respite pay rate is currently $9.00 per hour and Habilitation starts between $10.00 and $12.00 per hour - up to $14.00 per hour, based on education and experience. Habilitation Communication, Habilitation Behavioral and Consulting service pay rates are dependent on education and experience.
